As a New Patient of West Georgia Urology, we would like to welcome you and assure
you that the physicians and staff are committed to you and your well-being.

In order to make your First Visit with us as pleasant as possible we would ask your help
with the following:

Please arrive 30 minutes prior to your appointment (unless instructed otherwise by our
staff)

Also, please bring the following items with you to your appointment.

Current Patients

In order to make your visit to our office as pleasant as possible we would ask your help
with the following:

Please arrive 15 minutes prior to your appointment (unless instructed otherwise by our
staff)

If you have not updated your Information Sheets in the last 12 months you will need to
do so. You can click on the Forms and download the Current Patient Forms.

Also, please bring the following items with you to your appointment:

All insurance cards

Picture I.D.

Referral, if needed by PCP

Any x-rays, labs or other medical records

List of ALL medications you are currently taking with strength & dosage indicated

Please come prepared to pay your portion of your bill at the time of service. For your
convenience we accept Cash, Check and Visa or MasterCard.

Co-pays are collected at Check-In (before you will see the physician) and if self-pay or
co-insurance, your method of payment (check, cash, credit card) is verified at check-in
and paid at Check-out.

As a courtesy West Georgia Urology will file your Primary/Secondary Insurance but if
after 60 days payment from your insurance company has not been received, the balance
will be due and payable from the patient (see Patient Financial Agreement).

In order to minimize wait time for our patients, it is the policy of West Georgia Urology
that any patient who is more that 15 minutes late for their appointment be subject to being
Rescheduled. Patients who No-Show (don’t give at least 24 hour notice of cancellation/
reschedule) are subject to a $25 fee and chronic No-Shows may be subject to dismissal
by their physician.
